Calvary's Nursing Home Hospice began as a pilot project in a single nursing home in Manhattan, with the goal of bringing our comprehensive end-of-life care to residents. These patients are among our most vulnerable: they are elderly and from lower socioeconomic strata. Calvary@Home offers hospice services with 27 nursing homes to date, in Brooklyn, Manhattan, Queens, the Bronx, and Westchester, Rockland and Nassau counties.
Our Nursing Home Hospice program increases access to comprehensive hospice care and provides appropriate care to nursing home residents who are in the final stage of life. Our target population is nursing home residents with a prognosis of six months or less to live, as determined by a physician. Most members of this group are elderly men and women (age 65 and over) who suffer from a wide variety of illnesses, with the most prevalent being dementia, and also including end-stage heart disease, liver failure, cancer, and emphysema.
We view Nursing Home Hospice patients as needing the same level - and continuity - of care as community patients. We assign permanent staff to specific patients to ensure consistency and maximize comfort for the patient and family.
Since this program began, we have been able to break the anguishing cycle of repeat ER admissions for the patient and family that all too often marks the end of life for nursing home residents.
