Calvary Hospice provides end-of-life care to patients with life-limiting illnesses whose care has transitioned from active curative to palliative and quality of life focused. The majority of care is provided in the patient’s home and as needed, short term inpatient care is provided at several facilities throughout the greater New York area.

Maintains and coordinates the flow of information received in the referral process. Communicates with patient, family and appropriate staff to ensure accurate and timely insurance verification process. Is responsible for maintaining data entered in to the computer system.
1. High School Diploma is required.
2. Minimum of four years experience in a coordinator or related capacity is required. Experience in a Hospice, Home Health Agency or Hospital is preferred.
3. Competency with computers is required. A high level of competence with computer software such as Word and Excel as well as medical terminology.
4. Experience in intake and customer service activities is preferred.
5. Excellent telephone etiquette required.
6. Bi-lingual language skills preferred.
7. Ability to maintain confidentiality and highly developed organizational skills.
8. Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.
